West Ham eased to a 3-0 victory over Cardiff on Monday to seal a 5-0 aggregate win and book their place in the Championship play-off final.
Already leading 2-0 from the first leg in Wales, West Ham extended their advantage when Nolan headed home from close range on the quarter-hour.
Ricardo Vaz Te made absolutely sure there would be no slip-ups when he scored West Ham's second of the afternoon before half-time.
And Nicky Maynard rounded off the scoring in fine style with a thumping finish in the final minute.
West Ham will face Blackpool or Birmingham in the Wembley final on Saturday May 19.
